You Can Only Pick One

If you’re here for my opinion on the choice between universal healthcare and freedom, then you can navigate away. I’m here to showcase a poorly designed form:

… but in reality you can pick both. According to the W3C HTML 4.01 recommendation:

Several checkboxes in a form may share the same control name. Thus, for example, checkboxes allow users to select several values for the same property.

Looks like someone should have used radio buttons. Radio buttons are mutually exclusive.
